Posted February 26, 2013 under Dermal Fillers - No comments

There are over 300 dermal fillers that are available through out the world. In the USA the common dermal fillers are Juvederm, Restylane, Perlane, Prevelle Silk, Belotero, Radiesse, Silikon 1000and Artefill. This article will summarize the difference between the fillers to help you make your choice.

Dr. Sinclair's Response

When you are shopping for Dermal fillers, you need a basic understanding of the differences to help make your selection. Take this article with you and discuss it with your doctor before you make your final selection. Keep in mind the single most important issue in making your choice is what the injector is good at using.

  • Juvederm: It's made out of Hyaluronic Acid which is safe, common and very soft. Juvederm has the highest concentration of Hyaluronic acid at 24 mg/ml. It tends to last the longest of all of the Hyaluronic Acid Fillers. The higher concentration can cause more swelling. If you get a lump, Juvederm can be dissolved with Hyaluronidase. Juvederm Ultra is thick, Ultra Plus is even thicker and may last even longer.
  • Restylane: It is also made from Hyaluronic Acid. It has a lower concentration, 20 mg/ml. It is a bit stiffer which can help hold it's shape a little better. It tends to cause less swelling which makes it a good choice for use under thin skin such as the tear troughs. It can be dissolved.
  • Perlane: Is nearly identical to Restylane, but the particles are bigger. It tends to be thicker and may last longer. It can cause more swelling. It is good for deep injections. It can be dissolved.
  • Prevelle Silk: It is made out of Hyaluronic Acid, but at a much lower concentration, 5.5 mg/ml. It tends to not last as long. It is very soft and it can be dissolved. It is usually less expensive.
  • Belotero: It is made out of Hyaluronic Acid. It is the only Hyaluronic Acid filler that is made to be injected into a wrinkle. It is very good for use under thin skin such as the tear trough because it does not give a bruised appearance (Tyndall Effect). It can be dissolved. If it is injected too deeply, it tends to disappear very quickly.
  • Radiesse: It is 30% synthetic powdered bone and 70% carrier gel. It is very thick and holds its shape very well. The full size tubes are 50% larger than the other fillers, about 1.5 ml per tube. It builds up more collagen than Hylauronic Acid Fillers. It is too thick to be used in areas of thin skin. It should not be used in the lips because it tends to form lumps. It can not be dissolved but it will dissolve by itself over time. It only lasts 3-4 months on the first treatment, but much longer on follow up treatments in the same area. If you don't like the results, they will eventually go away without treatment.
  • Artefill: It is made out of microscopic plastic beads surrounded by collagen derived from a cow. It can cause allergic reactions, you need to be tested before you have it injected. It is permanent and can not be dissolved. If you get a lump it may never go away. It's predecessor, Artecoll, had a problem with causing sterile abscesses years after the injection. The incidence of sterile abscesses with Artefill is much lower but not zero.
  • Silikon 1000: It is made out of pure silicone. It is FDA approved for use inside the eyeball, all cosmetic uses are off label. It is very soft and it is permanent. It cannot be removed or dissolved. It needs to injected slowly over time using a microdroplet technique. It produces a very natural and soft improvement. If you don't like the results you will have to live with it.

In summary, there are lots of good choices for fillers, but no perfect choice. You should take the time to have your doctor help you choose the right filler for you.
